PDF form fields
The template includes fields for contact details, address, reference and date, plus two checkboxes. Supported controls stay interactive in PDF exports.
Labels are separate text objects and can be renamed without changing the fields.

Collect only what you need
Remove unused fields and label each expected answer. Require a field only when the process cannot continue without it.
For personal or sensitive data, tell respondents who receives it, why it is needed, how it is stored and where to ask questions.
Test the PDF
Open the exported PDF in another reader and complete every field. Check the tab order, multiline inputs, checkboxes, signature area and printing.
