Record the actual workplace
The first page identifies the organisation, activity, assessor, people affected and immediate actions. The second page provides four large hazard rows with multiline form fields.
The HSE warns against copying an example and adding a company name. The assessment must reflect the workplace's hazards and controls.

Make actions specific and accountable
Describe each hazard clearly, identify the people exposed, and record controls that actually operate. A further action needs an owner and realistic deadline so it can be followed up.
Add pages or rows when four hazards are not enough. Do not compress important information into unreadable type.
Review when work changes
Check the assessment when equipment, people, substances, layout or methods change, and after an incident or evidence that a control is not working. Keep approved versions according to your organisation's process.
